Below you can learn about our participant referral process. Our approach is different and so are our outcomes! Our services are not appropriate for individuals who exhibit the following:

  • Behaviors that would threaten or pose a health and safety risk to themselves or others
  • Severity of symptoms requiring a more intensive level of treatment
  • Disruptive behaviors
  • Behaviors that require excessive redirection and/or monitoring

Admission Criteria

1. Must have a primary diagnosis of a serious mental illness or documented disability. If your disability is not documented, you will be referred to our case management team for assistance.

2. Must indicate an interest in rehabilitation/recovery goals (employment, education, housing, or social network goals).

3. Must be 18 years of age or older.

4. Must be able to independently meet self-care and safety needs. All individuals are free to come and go as they like, they must possess the necessary safety skills, judgment, and/or tools to safely navigate the community without constant or one-on-one supervision from our staff.

5. Cannot pose a threat to others or themselves. This includes the risk of physical violence as well as property damage.

6. Must live in Washtenaw or a surrounding county.

7. Must have completed a tour with a staff member at least 30 days prior to the anticipated admission date. To schedule a tour, please phone 888-759-4917.

The Admission Process

1. Call 888-759-4917 to schedule a tour. Tours are typically scheduled on Mondays and Wednesdays at 11:00 am and 3:00 pm. Please note that tours are currently offered in person only. An assessment will be conducted to gauge potential interest and fit with our program.

3. If after the tour, the individual is interested in becoming a participant, the case manager, family and prospective participant will complete the Continuum Help Foundation Referral.

4. Fill out our online form or fax your completed application to 734-547-3014 along with documentation of diagnosis or disability. Fax documents to the attention of our Business Office Manger.

5. Our Business Office Manager will contact you within two weeks from the date of your tour to discuss admission status and/or the date of your orientation. Your referring source and family will also be notified of the outcome of their referral.

6. During orientation, a comprehensive Work Plan which reflects recovery goals will be developed.

7. After your plan is created, you will choose your work days and times and begin to work on your customized Work Plan.

8. An authorization request and Work Plan will be submitted to the appropriate referral source as required.
